Transcript Request - Other Options


Option 1:   If you do not have a working copy of Adobe Acrobat Reader 3.0 or above, click here to download Adobe Acrobat Reader from the Adobe website. Then, try downloading the Transcript Request Form again.

Option 2:   If you encounter problems using Adobe Acrobat Reader, click here to print an alternative Transcript Request Form.

Option 3:   You may fax written requests (showing your name, social security #, class year, and signature) to the Registrar's Office fax line: (609)258-6328. To specify special instructions for mailing the transcripts, see the Option 4, below.

Option 4:   If you prefer to send a request in writing (without the official form), show your name, Social Security #, class year, and signature. You may include these special handling instructions -
  • Place each transcript in an individual envelope.
  • Add the Registrar's signature on the outside flap of the envelope.
  • Expedite delivery of the transcripts by FedEx, etc. -- for which be sure to include your credit card # and expiration date(month/year), or your FedEx account number.
  • Then, mail your request to:
